01/ When is the earliest I can see you?
Please fill in the Appointment Request form and we will contact you to make the appointment and do our best to provide you with an earliest possible timeslot.
For medical emergencies please always call 000.
02/ What do I need to make an appointment?
To make a new appointment please have the following available:

Medicare card if you have one
-
A referral letter from your GP or Specialist
To make a followup appointment, please ensure that your referral letter is still valid. A referral letter issued by a GP is valid for 12 months and one issued by your Specialist is valid for 3 months.
03/ What do I need to bring to the appointment?
Please bring your:

Medicare card (if you have one)

Referral letter

Any recent pathology results such as blood tests, x-rays.
04/ Can I take public transport to your location?
We are conveniently located in Chatswood about 50m away from the train station. If you prefer to drive then you can park at either the Westfield car park or at InterPark Australia on Brown St, Chatswood 2067.
05/ Do you provide Telehealth consults?
Yes. Please indicate your preference on the Appointment Request form. Please note that pre-payment is required for Telehealth consultations.
